Why

The NHS Plan identified the need for changes in the way in which patients are asked to give their consent to treatment, care or research, in order to ensure that the process becomes properly focussed on the rights of individual patients and their relatives. The Department of Health has recently issued revised guidance regarding obtaining informed consent together with model consent forms.

The aim at Addenbrooke’s NHS Trust is to ensure that patients are provided with relevant and understandable information regarding their procedure and that appropriate consent is obtained in a timely, informed, consistent and comprehensive manner.

What

The information contained in this website covers those procedures requiring written consent. This is under continuous development, and more procedures will be added in due course.

Each consent form contains details of:

  • What the procedure involves
  • What to expect before, during and after the procedure
  • The risks and benefits
  • Any alternatives that are available
