In this section, you can download consent forms which you are required to complete prior to investigation or treatment undertaken at Addenbrooke's.

The Addenbrooke's consent forms (and information) will be given and explained to you before your procedure.

They are published from this website in Adobe Acrobat PDF format - so you can view them/print them and read up on them in advance, if you wish.

In order to open them you will need to download and install Adobe Reader.

How to find the right consent form

You can either:

  • Use the Consent Forms by Procedure Name page if you know the name of the procedure

  • View and print off the generic forms (including for postmortems and retention of organs): see below.

Some Addenbrooke's services use the generic forms rather than procedure-specific forms

 

Generic forms

Consent form 1 - Patient agreement to investigation or treatment

Consent form 2 - Parental agreement to investigation or treatment for a child or young person

Consent form 3 - Patient/parental agreement to investigation or treatment (procedures where consciousness not impaired)

Consent form 4 - Form for adults who are unable to consent to investigation or treatment

Consent form 5 - Consent to surgical treatment by patients who refuse to have a blood transfusion ('Jehova's witness form').

The Department of Health publishes information about these forms, including versions in languages other than English.

Consent for post mortems

Addenbrooke's now uses postmortem consent forms published by the Department of Health (DH) and given (with other DH information) to relatives by staff of Patient Affairs and/or Children's Services. The consent forms on the DH website are for information only. They are not for printing and use within the Trust because the forms are a legal document and specially designed in triplicate.
