Enabling continuity of care

Follow-up visits are undertaken for all patients discharged from the General Intensive Care and Neurosciences Critical Care by Dr G R Park and Dr R Burnstein.

These visits are an opportunity for the outreach team to ensure that the patients’ transition to the ward has been straightforward enabling continuity of care. They also allow patients and relatives the opportunity to discuss their intensive care stay, allowing any anxieties and comments to be expressed and resolved.

Patients are given the opportunity to re visit the intensive care unit enabling them to dispel any distorted memories they may have.
